At this time, our services are in particularly high demand so we ask for your patience as it may take up to 3-5 business to receive a call from one of our experienced patient care coordinators.

Once you have an appointment booked, I will need any relevant prior records. Most importantly, it’s very helpful to  me if you fill out your health history on our portal prior to your visit. Doing so allows me to get to know you better before the visit, be prepared, and save valuable time so that we can spend our visit discussing testing and treatments. Our patient care coordinator will explain how to do all of this when we make your appointment.
